The terminal and most useful weapon for the treatment of impotence

〜 Penile prosthesis implantation 〜

Impotence (male erectile dysfunction) is very common problem in the middle and aged male. Although oral phosphodiesterase type 5 inhibitors, lower energy shock wave therapy, vacuum erection devices, and intracavernosal vasoactive agent injection are the preferred therapies for erectile dysfunction (ED), penile prosthetic implants are regarded as effective, safe, and durable for patients who are refractory to medical treatment and/or prefer a more effective and permanent therapy.

From October, 2000 to September, 2022; 45 impotent patients underwent penile prosthesis by Professor Ying-Huei Lee at Kaomei Urologic Clinic (the most famous and specialized Urologic Clinic in the Central and Southern Taiwan).

【Patients profile】

Average age: 60 ± 10 years old (range from 37 to 86).

Associated medical morbidities:

1. 1)None, simple erectile failure (ED) 18
2. 2)ED + diabetes mellitus 18
3. 3)ED + post radical prostatectomy 6,
4. 4)ED + hypertension 11,
5. 5)ED + coronary disease 5,
6. 6)ED + hepatoma 1.

Average hospital day: 1 day (98 % of patients discharged within 21 hours).

【Single piece penile prosthesis】: 25 patients

Average operation time: 56 ± 14 minutes (range from 34 to 95)

【Two pieces penile prosthesis】: 11 patients

Average operation time: 88.9 ± 9 minutes (range from 75 to 103)

【Three pieces penile prosthesis】: 9 patients

Average operation time: 144 ± 22 minutes (range from 122 to 158)

There were no serious side effects, one patient received three pieces penile prosthesis needed to revise the penile prosthesis due to mechanical failure, another one needed to revise the pump position.
